Submitting Medical Information

For all physicals, a link for a patient intake survey will be sent to you prior to scheduling your physical. This will ask contact information, diving history, medical history and full medication list. Please make sure all details are correct as these will be used to complete any necessary clearance documentation.
 
Additional required information is listed below for each type of physical. These records can be submitted by email before your consultation.
 

Recreational Diver Physical

Do you need medical clearance for an upcoming basic open water course or sign off on a Divers Alert Network (DAN) form?

•  Divers Alert Network (DAN) physical form - link here DAN Medical Form - please fill out initial history form prior to your consultation

• Have a medical condition? - please provide visit notes from your primary physician or consultants in the past 12 months
• Diabetic? - will need your last 2 hemoglobin A1C
• Asthma? - any pulmonary function test (PFT) in the past 5 years
• At risk for heart disease? (high blood pressure, high cholesterol, age >50)? - Any recent EKG, stress test or lipid/cholesterol profile

 
Commercial Diver Physical

Do you work as a commercial diver or looking to attend dive school? I am here to help. I can complete all the extra tests you need as well as communicate with your company for any questions about your medical issues.

 

The standard exam in the United States is the ADCI physical and all physicals will comply with those standards. Below is a list of things you will need:

• Are you renewing your annual physical? - please provide your initial physical and your most recent physical
• Have a medical condition? - please provide visit notes from your primary physician or consultants in the past 12 months
• Chest X-ray - within 3 years- either provide previous xray report or I can order new one
• Age >35 years - need EKG and lipid panel within 12 months. Can be ordered by myself

 

Commercial Physicals generally require the following tests to meet ADCI standards:

• Completed in-person during your consultation
  • History and physical
  • Spirometry (PFT)
  • Audiogram
  • Visual Acuity
  • Color vision
  • Urinalysis
  • Age >35 years? - EKG
• Blood tests within 12 months (or can be obtained at local lab)
  • Complete Blood count
  • Age >35 years? - lipid panel (blood tests)
• Chest x-ray - initial and every 3 years. If >3 years since last CXR, can order at local imaging center

Scientific Diver Physical

Requirements will depend on the type of physical being requested. The most common standard is AAUS however many universities use their own custom exam or requirements. Please provide your required paperwork and any other relevant medical information listed under "Recreational Diver Physical"

 

Public Safety Diver Physical

Requirements vary by team, so check with your leadership. Most teams choose to have minimal specific disqualifying conditions and leave up to physician discretion. We are happy to comply with whatever standards you need and help you get back in the water safely.
