Submitting Medical Information
For all physicals, a link for a patient intake survey will be sent to you prior to scheduling your physical. This will ask contact information, diving history, medical history and full medication list. Please make sure all details are correct as these will be used to complete any necessary clearance documentation.
Additional required information is listed below for each type of physical. These records can be submitted by email before your consultation.
Recreational Diver Physical

Commercial Diver Physical

Scientific Diver Physical
Requirements will depend on the type of physical being requested. The most common standard is AAUS however many universities use their own custom exam or requirements. Please provide your required paperwork and any other relevant medical information listed under "Recreational Diver Physical"
Public Safety Diver Physical
Requirements vary by team, so check with your leadership. Most teams choose to have minimal specific disqualifying conditions and leave up to physician discretion. We are happy to comply with whatever standards you need and help you get back in the water safely.
